if you SURVIVE a covered medical crisis, OR pays your
family up to $1,000,000 if you die. Enabling
you and your family to choose, allows peace of mind
during your time of need.

1. Heart Attack: The death of a portion of heart muscle.
2. Stroke: With evidence of a permanent neurological
deficit.
3. Invasive Cancer: Characterized by the spread of
malignant cells, including malignant melanoma.
4. Kidney Failure: End-stage renal failure requiring
dialysis or hemodialysis.
5. Blindness; total, permanent loss of vision in both
eyes, * (benefit not available in South Carolina)
6. Paralysis: Complete and permanent loss of two or
more limbs.
7. Major organ transplant of a heart, lung, liver,
kidney, or bone morrow.
8. Non-invasive cancer, a localized malignant tumor
with the potential to metastasized, pays 10% of benefit. If
it spreads, you will receive the balance of the benefit.
9. Terminal Illness with a life expectancy of 12 months
or less.
If one of these medical emergencies happens to you and you
SURVIVE: You get a lump sum CASH payment upon diagnosis, to
spend any way you see fit.
If you die, from any eligible cause, your family receives
the benefit.
